The DICOM image decoder fails to validate that pixel indices are within the bounds of the lookup table size.

Description
A malicious DICOM image containing out-of-range palette indices can trigger out-of-bounds read from heap memory during image rendering.
Impact
- Out-of-bounds heap read during image rendering.
Mitigation
Update Orthanc to version 1.12.11 or later.
